Home Care

CCMH – Home Care is a hospital based Medicare Certified Home Care Agency.    Home Care has been in the top 25 percent nationally since 2007 for patient outcomes

Home Care is the whole range of health care services provided to persons in their own homes.   Home Care offers a way of supplementing the care received in the hospital or clinic.   It bridges the transition from being acutely ill to being independent or to maintaining your independence.

Studies show that people recover faster when they’re in their own home.   Many treatments which used to require hospitalization can now be administered at home by a qualified health professional.

Home Care promotes family integrity and independence to the individual needing care.   Each caregiver is trained to provide individualized care with a personal touch.   Using Home Care’s services contributes to a better overall quality of life for the family by helping to relieve some of the stresses associated with caring for a loved one.

Working with your physician, Home Care staff will come to your home to provide prescribed medical care.
