HOURS:                        0.6 FTE (potential to increase hours)

Education/Training:  Graduate of a state-accredited school of nursing with Associates, Diploma or Bachelor’s Degree in Nursing. Must possess and maintain current Minnesota license as a Registered Nurse.  Certified in OASIS assessment preferred.



  • Ability to deal with problems involving several variables in standardized situations.
  • Has reliable transportation to client’s home as assigned.
  • Must be able to demonstrate effective communication on a one-to-one or in small group settings to client, customers, and other employees of the organization.
  • Must be punctual, attendance satisfactory and provides proper notice of absence or tardiness.
  • Must be able to handle conflict in diplomatic and tactful manner.
  • Must be adaptable and flexible to changing situations and to client schedules on a daily basis.
  • Must be able to prioritize and organize work efficiently to accomplish workload in time allotted.
  • Must be able to maintain national home care standards with productivity.
  • Must work as a team member.
  • Willing to work with others and cooperate fully for the good of all.
  • Must maintain a positive attitude.
  • Attends mandatory in-services and education as required by statutes.
  • Must follow safety rules/regulations of the agency, reporting injuries and accidents to managerial staff.
  • Knowledge in ICD 10 Coding
  • Must be organized and able to function independently with little orientation to the Home Care setting.

Experience:  At least two (2) years of nursing experience in either acute care or home care setting with point of care computerized charting, within the past two years preferred.  OASIS, coding, Medicare case management experience preferred.
