CCMH offers diabetes education and assistance with diabetes management for:
- New Diagnosis of Type 1 or Type 2 Diabetes
- Gestational Diabetes
- Secondary Diabetes
- Pre-Diabetes
- Complication Prevention
- Medical Nutrition Therapy, including:
- Weight Management
- Hyperlipidemia (Cholesterol disorder)
- Hypertension (High Blood Pressure)
- Food Allergies
- Eating Disorders
Group classes and individualized classes are offered on education and management of your diabetes.
While these services have been offered at CCMH since 2006, our educators’ combined experience totals more than 37 years.
The Chippewa County – Montevideo Hospital’s Diabetes Education Program is recognized by the American Diabetes Association.
Our services are covered by most insurance providers, Medicaid, and Medicare.
